Donor Family Quilt
 

Southwest Transplant Alliance invites you to participate in the
Donor Family Quilt Project.

This quilt is dedicated to the memory of your loved one, who gave the Gift of Life.
Each quilt square commemorates an individual who saved others through organ donation.

The quilt is displayed throughout Texas to help spread the message of organ and tissue donation, and to remember those who gave. Families are encouraged to share their
feelings (about 100 words) about making the square and may include comments about
the quilt square's design. These are being compiled into a book.

Quilt patches should be 8" square, including a 1/2 margin, any color of fabric. You may
use crayons, paint, markers, thread, or any other medium to design the square. Please
feel free to include the name of your loved one and dates of birth and death, if you wish.

Please send squares using Insured Carrier or Return Receipt Requested.

Please be sure to include your name, address and phone number with the
square. You will receive a letter confirming the receipt of the square in 6
to 8 weeks. Please notify Southwest Transplant Alliance if you do not
receive confirmation.
